Does the inner sleeve shown below need to be removed before installing the lower half of the bushing, or do I just need a BFH?

Even after letting the bushing sit in the freezer overnight, it still doesn't appear to want to fit without trimming...

I just did mine and had the same issue. I had to take the sleeve out. I pounded it out from the top. There is no way you will ever get the poly mount through there. The poly mount has the same outside dimensions as the sleeve. It would need to be smaller to get in there

it doesn't take much to pound it out, I just put a wood block on it and hit it with a hammer a couple times and it fell right out. It doesn't get damaged.

I would like to see someone else chime in here and give an opinion. I really don't see how anyone can use that metal sleeve though. To use it you would have to cut apart your poly bushing to make it smaller for the fit.

Be careful though. Check for rust between the two plates on the flat mounting points of the cradle. On my cradle, there was enough built up rust and grime flakes that it was pushing the entire plate up and deformin the top surface. I had to grind the thin plate's welds off, remove the rust (wirebrush/grind), and tack the plate back on. Just something to check for.
