Any type of event that is not part of the "normal" chain of events is referred to as an anomaly.

Very few with astronauts on board. Technically only one would be considered a "rocket explosion" out of the three U.S. manned spaceflight missions that have had fatalities.
Apollo 1 - fire in spacecraft. Rocket didn't explode, and it was during a test rather than the actual flight. 51-L - Challenger breakup after SRB & ET failure during ascent. STS-107 - Columbia breakup during re-entry.
Lots of unmanned rockets, though. Probably the most embarrassing of these was dubbed by the media as "Flopnik."
You might also consider the X-15 flight that killed Michael J. Adams since it was rocket propelled, but it wasn't a rocket explosion that led to the crash. At 230,000 ft altitude, he went into a spin traveling at Mach 5.
Tweaking the fuel injectors and the waste gate on that super charger may not have been a good thing. The technician said don't touch that knob............. He's just a technician, what's he know......

You can see the top portion that housed the payload blown complete off the Falcon at about 1:10. That portion also exploded when it hit the ground. I guess this will create a little room in their storage hangar, since it was getting full from the successfully recovered Falcons.

In 2014, SpaceX started regularly conducting static fire tests with the rocket's payload on-board, first with Dragon cargo capsules heading to the International Space Station, then with commercial satellites.
I work directly across the river from the launch pad... no busted windows on our building, but I heard some closer to the explosion didn't fair as well. Scary stuff, glad nobody got hurt or killed.
