The only times I have problems with it are:
- I must already be logged in. E.g., it will not work if I'm creating a new post (after having not logged in yet), and THIS is the post I'm trying to upload a picture to... where at the same time while posting this, I've also just now entered my username and password. Because the image loader hasn't yet been authenticated (no cookie yet exists), it craps out.
- I cannot upload a file if I know that the file already exists. If I say yes to go ahead and re-upload a file with the same name, it craps out (assuming some kind of file protection). So, I always name files with a different name when uploading so I don't run into that.

It is my understanding that we don't use" pip" anymore. Use the upload media button in the lower left hand corner

No... because most of us don't use it anymore.
