I can barely remove it with my hands but yet every time I open the hood I have to put it back on. What would be causing it to pop off all the time?

do you mean the vacuume hose? The EGR tube shold be bolted together. (V6)

Put a small wire tie around it and pull it tight.
Either that or go and purchase new rubber tubing to install.

When the rubber gets old and oily, it tends to swell and harden. Best solution is to just replace the tubing; it's cheap. I usually get short pieces like that EGR hose for free 'cause it's so cheap it's not worth writing up...
